Our $49 Medical Absence Verification is for work or school absences of up to three days. We can only provide it to people who are physically located in one of our licensed states when they submit their request.
Which state will the patient be in during the request?
We currently serve Arizona, Colorado, Florida, Iowa, Illinois, Kansas, Kentucky, Montana, Nevada, Oregon, Virginia, Washington and Wyoming.
Precision Medical Docs cannot yet provide services to people located in your state. We are working to expand and appreciate your interest.

Who is the work or school note for?
Services are available to people located in our licensed states at the time of their visit. Documentation is provided only when clinically supported.
Is the note for an illness, an injury, or another issue?
Precision Medical Docs provides work and school notes only for absences related to a qualifying medical illness or injury. Based on your response, we are unable to provide a note for this request.
